Teacher information, forms and timesheets


TALK is currently recruiting!

Do you sign (American Sign Language)?  Do you speak French or Spanish? 

TALK recruits teachers year-round. While we recruit our regular teachers in the summer, we are always on the lookout for substitute teachers who can be available to step in for our teachers during planned absences. We currently offer early-morning language classes at seven different locations in Davis. If you are fluent in one of these languages, and have experience working with young children (and you love it), please send us your resume at talkprogram@hotmail.com.

 

 

Prior to Employment: Forms

At Teacher Orientation and Training or at the time of hiring, you will be required to file the following forms prior to the start of your teaching duties:

Form I-9 "Employment Eligibility Verification" requires that we examine your proper documentation for legal employment in the US.  Instructions for completing the I-9 are here.

Form W-4 "Employee's Withholding Certificate" is required so your taxes are withheld properly.

The Direct Deposit Form allows our payroll service to deposit your paychecks directly into your account.

The Teacher Contact Information Form provides us with key information about how to reach you, and who to reach for you in case of emergency. Please fill this out and submit it online. If your information should change during the year, please use this form to update us.


Please remember to bring the following to Orientation:

- Proper government documents (see I-9 instructions)

- A voided check from your checkbook. If you do not have a checkbook, please ask your bank for a printout of your account information for direct deposit.

 

Background Checks

All teachers must undergo a LiveScan fingerprint and background check. You will be given a LiveScan form, which you will bring to PDQ Fingerprinting, a mailing and office services store located at 216 F Street, Davis.

All teachers must also complete the Safe Haven online course to help prevent and identify abuse. More information will be given at Teacher Orientation.

 

 

Important Forms

To report an injury that might have occurred in class:

use the online TALK Ouchie Report or fill it out in Word using the Ouchie Report Form and send to TALKprogram@hotmail.com

 

To report repeated negative behavior from a student:

fill out the Class Behavior Record Form in Word and send to TALKprogram@hotmail.com. 

 

To assess a student's progress:

At the end of each term, please use the TALK Student Evaluation Form. Fill out one form per student, and e-mail that form to both the child's parents and to TALKprogram@hotmail.com.
